WHAT DO WE ASK YOUR CLIENT?
Treatment Status: Provider names and contact information, frequency of treatment, kind of treatment, changes in diagnosis, other issues or incidents, treatment plans or recommendations and any other concerns you would like addressed.

Wage Loss: Any? How much? Employer information, income, duration of wage loss, duties of job, what duties are they unable to perform, expected return to work date, if no wage loss - how do your injuries impact your performance at work?

Other Impacts From the Loss: Emotional, family activities and relationships, ability to participate in recreational activities, hobbies, out of pocket expenses – how much and what for?
